Tracheal Resection for Advanced Thyroid Cancer

  • Locally advanced thyroid carcinoma manifests itself with gross extra thyroid extension of the primary tumor involving adjacent structures. Anterior extension to involve the strap muscles is classified as T3, while posterior extension to involve the trachea, larynx, esophagus and recurrent laryngeal nerve is classified as T4a. Accurate assessment of the tumor extent is essential to plan a complete resection achieving a R0 status. This often results in long term cure. The patient presented here has papillary carcinoma of the thyroid with extra thyroid extension involving the trachea and inferior constrictor muscle at the pharyngo esophageal junction. Detailed radiological evaluation and endoscopic assessment are essential for the feasibility of a R0 resection. The operative procedure demonstrates step by step the technique of tracheal resection and reconstruction of the airway.
